Ask HN: How does your company track issues and bugs from internal stakeholders?

My company uses Slack, Google Forms, and Clubhouse for issue tracking. The problem is it’s difficult to get good information on the initial submission and often requires additional follow up. I was wondering if anyone knows of a process that works well for their company.
